This webinar featured a multidisciplinary team from Northern Sydney Local Health District (NSLHD) discussing current practice and developments in Hospital in the Home (HITH) services across the district.

The presenters outlined how HITH models are being applied across NSLHD to support acute and sub-acute care in the community. They explored patient selection, escalation pathways, collaboration between specialty services, and the role of nurse practitioners in advanced clinical decision-making. The session also highlighted case examples demonstrating integration between intensive care, hospital medicine, and HITH services, and discussed opportunities for service development.
This event provided practical insights into how multidisciplinary teams can deliver safe, patient-centred care in the home while supporting hospital capacity and patient flow.
