Comprehensive care: from acute inpatient to cancer care
The Monash Health team presented their model of Adult HITH care spanning infectious diseases, haematology, oncology, and palliative care. They described how patients transition safely from inpatient wards to home-based care, supported by multidisciplinary collaboration and clearly defined clinical pathways. The session highlighted governance structures, escalation processes, and examples of complex case management, demonstrating how HITH can extend specialist-led care beyond the hospital walls.
The presentation illustrated how comprehensive HITH services can improve patient experience, maintain clinical safety, and create capacity within acute hospitals while supporting people to receive care in their homes.
