This Grand Round brought together two leading services to showcase different models of Hospital in the Home (HITH) care designed to safely manage complex patients in the community and reduce demand on Emergency Departments.


Alfred Health, Victoria

A HITH-led ecosystem for complex patients


Alfred Health presented their HITH-led model of care focused on supporting high-risk and complex patients outside the acute hospital setting. Their presentation demonstrated how an integrated ecosystem, working closely with community providers, can reduce Emergency Department presentations and avoid admissions through proactive, coordinated care. The team discussed patient cohorts, governance structures, and outcomes achieved through this approach.
